Testing is now available for anyone interested in a test. To schedule an appointment for COVID-19 testing and curbside care at Frances Nelson, call our COVID-19 Care Line at (217) 403-5402.
Promise offers both viral and antibody testing. Testing will include a medical appointment--covered by most health plans and our sliding fee scale. - Most dental services are now available with SmileHealthy.
- Promise at the CRC has been moved to Frances Nelson throughout OSF's additional COVID-19 screening period.
- Urbana School Health Center will be closed and care delivered at Frances Nelson but plans to reopen in July.
- You will be asked to complete a screening form prior to scheduling an appointment and when you arrive. This will assist us in directing you to the most appropriate assessment or treatment location.

Food As Medicine - Emergency Food Program At Frances Nelson and our Community Resource Center satellite at Presence Covenant, Promise Healthcare provides nutritious food to prenatal and pediatric patients that need food resources. Each bag provides 2-3 days of nutritious food, health information, and information about food pantries near your home. Ask your doctor about the program today.

Frances Nelson is a primary care medical and mental health clinic dedicated to providing affordable and accessible high quality health care services to the medically underserved population of Champaign County now with a walk-in clinic and convenient care access.
The
SmileHealthy Dental Center is dedicated to providing excellent oral health care to the underserved community. We also have a mobile dental program, provide dental care at area Head Start schools, pediatric restorative outpatient surgery, and dental health education.
Chronic Care Management and Case Management - Promise Healthcare nursing and case management staff reach out to interested patients monthly about medications, referrals, upcoming appointments, community resources and more. Ask your provider about how to add this support to your care.
Promise Healthcare and our programs are in-network for Molina, Medicaid, Medicare, Health Alliance and most commercial insurance. A sliding fee discount is available for income eligible patients. This health center receives Federal funding including funding and/or cooperative agreements from HRSA. In 2019 Promise Healthcare's budget was $5,933,767.
COVID-19 Charges
Promise Healthcare will bill health care coverage for those seeking care and testing for COVID-19. Promise follows the Families First Act--there will be no patient responsibility for care that meets medical necessity. Promise charges for visits that include our COVID-19 care are 99203 new pt $231.00, 99202 new pt $177.00, 99213 established pt $170.00, and 99212 established pt $150.00. Laboratory charges are not included. Promise Healthcare is using the Carle laboratory for COVID-19 testing. Carle laboratory rates are $90 to $195 and subject to chage. Contact Carle to confirm. Patients should not have out of pocket costs for either the visit or lab for visits that meet medical necessity including those with potential exposure and/or with symptoms.
