Promise Healthcare Achieves Patient-Centered Medical Home (PCMH) Recognition from NCQA
Champaign, IL— Promise Healthcare, a local Federally Qualified Health Center (FQHC), is proud to announce that it has achieved Patient-Centered Medical Home (PCMH) Recognition from the National Committee for Quality Assurance (NCQA). This recognition reflects the organization’s commitment to delivering high-quality, coordinated, and patient-centered care to the communities it serves.
The National Committee for Quality Assurance PCMH Recognition program is a nationally recognized distinction that validates primary care practices for their focus on quality improvement, care coordination, enhanced access, and team-based care. Practices that earn this recognition demonstrate the ability to provide comprehensive, patient-focused care that improves outcomes and strengthens the patient experience.
“Achieving PCMH Recognition is a significant milestone for our organization,” says Jennifer Henry, Chief Executive Officer. “It reflects the dedication of our care teams to delivering whole-person care that is accessible, coordinated, and focused on the unique needs of each patient.”
As a recognized Patient-Centered Medical Home, Promise Healthcare:
- Provides team-based, coordinated care across the healthcare continuum
- Emphasizes preventive services and chronic disease management
- Uses data-driven quality improvement strategies
- Engages patients and families as partners in care
- Enhances access through expanded services and communication options
PCMH Recognition aligns with the health center’s mission as an FQHC to reduce health disparities and improve access to care, particularly for underserved and vulnerable populations. The recognition underscores ongoing efforts to integrate behavioral health, address social drivers of health, and ensure patients receive comprehensive, culturally responsive services.
“Our patients are at the center of everything we do,” says Melonie Richardson, Chief Medical Officer. “This recognition affirms that our systems, workflows, and care teams are structured to support better health outcomes and a better care experience.”
