Our Community Health program takes our care beyond our clinic walls, providing vital enabling services to Champaign and other nearby counties.

We provide critically important non-clinical, enabling services that help reduce barriers to care and increase access to health care and social services for our patients.

These enabling services include COVID-19 resources and education, rent and utility assistance referrals, assistance accessing healthcare coverage – including Medicaid, health education, referrals, case management, and more.
